What made this surgery particularly challenging was the patient’s diagnosis of situs inversus, a rare congenital condition occurring in approximately 1 in 10,000 individuals, where the internal organs are positioned in a mirror image of the normal layout. In this patient, his liver and gallbladder were on the left side instead of the right, while the spleen and heart were on the right, creating a completely reversed anatomical scenario for the surgical team. The Patient was admitted at Fortis Escorts Faridabad with severe abdominal pain, along with nausea and vomiting. He had a prior history of Type 2 diabetes and coronary artery disease and had undergone angioplasty. His heart’s pumping ability was significantly weakened, with only 30% efficiency compared to the normal range of 50 -70%. An ultrasound was conducted at the hospital, which revealed acute calculus cholecystitis, along with signs of biliary peritonitis – bile poisoning, which could quickly lead to multi-organ dysfunction, if left untreated.
Given the patient’s narrowed heart arteries, he was on blood thinners to prevent clots, increasing the risk of excessive bleeding during surgery. After a carefully executed two-hour procedure, the severely infected gallbladder was successfully removed, preventing a life-threatening situation.
